Registration Information
All registration fees and forms must be completed directly through the Recreation & Community Services Department. You may register online, over the phone at 414-604-4900 with a credit card or mail or drop off at 2450 S. 68th Street.

Athletic Code of Conduct Agreement

Student Athlete Emergency Information Form (only needs to be done once per school year).

Physical Exam Form (good for 2 years - Prior to April 1, 2023)

Physical Exam Alternate Year Card (must be filled out the following year after physical exam is completed - physical on or after April 1, 2022)

Concussion & Sudden Cardiac Form (only needs to be completed once per school year for intermediate sports).

PHYSICAL EXAM ALTERNATE YEAR CARD
We're thrilled to announce that we're moving our Intermediate Sports forms to a paperless electronic format! This change is designed to simplify the process, making it more convenient and efficient for everyone.Starting in the 2024-2025 school year, all sports forms will be accessible online through CampDoc. You can upload and complete these forms and submit them electronically, reducing paper waste and providing faster access.A link to CampDoc will be sent via email after registration is completed through the WAWM RCS Department. We appreciate your support as we make this transition and look forward to an exciting sports season ahead!
Athletic Opportunities
Poms/Cheer: September – End of January

Tryout Dates: early September

Practice will be once per week determined by coach.

Athletes will perform at halftime of the girls and boys basketball games.

Athletes will compete in 4-5 meets

Tryouts will be held in early September for 7th and 8th Graders - Dates TBD.

Tryouts will be held late October/early November for 7th and 8th Graders - Dates TBD.

Season may overlap with wrestling but participants are allowed to compete in both. We ask that you communicate your interest with your schools on-site Athletic Director.
